Abstract:

The research was made for the City Health Office of Calamba which is under the Department of Health. The system made in the PHP programming language complied with the objectives and needs of the client. It would benefit the client in terms of handling information, in adding patients’ record and retrieving data. It has a computerized database to help the City Health Office in their everyday processes.
